Medical Practitioners can easily refer by sending an email to intake@healthinmindservices.com.au

Please provide a patient summary that includes their physical and mental health history, a medication list, and any risk concerns.

Please note – Nurse Practitioners do not require a Mental Health Care Plan to deliver services, however that additional information would be helpful to include in the referral.
