Referrals
Medical Practitioners can easily refer by sending an email to intake@healthinmindservices.com.au
Please provide a patient summary that includes their physical and mental health history, a medication list, and any risk concerns.
Please note – Nurse Practitioners do not require a Mental Health Treatment Plan to deliver services, however that additional information would be helpful to include in the referral.
Patients can self-refer to Health in Mind and schedule an appointment. Prior to your initial appointment, please obtain a referral from your General Practitioner and/or a Patient Summary which outlines your health history and medications. We require the details of your General Practitioner in order to deliver collaborative care. Please provide details of any other health professionals involved in your mental health care, such as Psychologists or Psychiatrists.
