Referrals
Patients can easily self-refer to Health in Mind and access Medicare rebates without the need for a GP referral or Mental Health Care Plan. There is no limit on the number of sessions that can be provided annually. After booking is confirmed, patients will be emailed an intake questionnaire which includes demographic information and brief enquiry into your physical and mental health history.
Medical Practitioners can easily refer by sending a standard GP referral through to our email. Please provide a patient summary that includes their physical and mental health history, a medication list, and any risk concerns. Health in Mind does not require a Mental Health Care Plan to deliver patient services and offer Medicare rebates. If a Mental Health Care Plan exists or is yet to be completed, that can be sent through alongside other relevant clinical information. Nurse Practitioners do not have an annual limit on the number of sessions that can be provided to patients.
